Job Overview
We are seeking a dedicated and experienced Primary Care Nurse Practitioner to join our team on a full-time basis. The ideal candidate will possess at least three years of clinical experience, demonstrating expertise in primary care settings. This role offers an opportunity to deliver patient-centered care across diverse populations, utilizing advanced clinical skills and a broad range of medical procedures to promote health and well-being.

Key Responsibilities

• Provide comprehensive outpatient primary care services
• Diagnose and manage acute and chronic medical conditions
• Perform preventive screenings and wellness visits
• Manage hypertension, diabetes, obesity, asthma, hyperlipidemia, and metabolic disorders
• Order and interpret labs, imaging, and diagnostics
• Prescribe medications and develop treatment plans
• Evaluate patients who may benefit from IV therapy as part of primary care treatment plans
• Collaborate with behavioral health providers for integrated care
• Document encounters in AthenaOne EMR within 24 hours
• Provide health coaching, patient education, and counseling on lifestyle modifications and medication adherence.
• Collaborate with multidisciplinary teams to coordinate care plans, discharge planning, and follow-up appointments.
• Stay current with medical advancements and maintain compliance with HIPAA regulations to ensure patient confidentiality.

Minimum Qualifications

• Master’s or Doctoral Degree in Nursing
• Board Certified FNP or ANP
• Active Maryland NP License
• National Certification (ANCC or AANP)
• Active DEA and Maryland CDS
• Minimum 3 years outpatient clinical experience preferred
• Strong documentation and EMR skills
