Primary Care Nurse Practitioner – Independent Contractor (1099)

Position Overview

We are seeking a compassionate, collaborative, and experienced Primary Care Nurse Practitioner to join our expanding provider team as an independent contractor. This is a 1099 position offering a dynamic mix of office days, mobile visit days, and telehealth days. You will work closely with our behavioral health providers, therapists, and facility staff to deliver coordinated, patient-centered care across a variety of settings.

This role is ideal for a provider who genuinely loves what they do, values teamwork, and understands that great outcomes happen through collaboration not in isolation.

What You'll Be Doing

  • Conducting comprehensive primary care evaluations and chronic disease management
  • Seeing patients across office, mobile, and telehealth settings on a rotating schedule
  • Serving patients in assisted living, skilled nursing, and other care facilities with professionalism and warmth
  • Collaborating closely with behavioral health providers and therapists for integrated patient outcomes
  • Building and maintaining strong relationships with facility staff, administrators, and patient families
  • Maintaining timely, accurate documentation in EHR for every encounter

What We're Looking For

  • Active Arizona NP license (FNP preferred)
  • Active DEA registration
  • Minimum 1 year of clinical experience (primary care or geriatric experience strongly preferred)
  • Experience working in facility-based settings such as ALF, SNF, or group homes is a strong plus
  • Genuine warmth and professionalism when interacting with patients, families, and facility staff
  • Strong collaboration skills you understand that working alongside behavioral health and therapy teams leads to better patient outcomes
  • Reliable, responsive, and detail-oriented documentation completed same day
  • CAQH profile or willingness to create one
