Overview

Evergreen Health Care Management is a rapidly growing healthcare organization providing Personal Care, Home Health, Hospice, Adult Day Health Care, and community-based healthcare services throughout Northern Virginia.

As part of our continued expansion, we are launching a Primary Care and Chronic Care Management (CCM) division to better serve our existing patient population and surrounding communities. We are seeking a compassionate, motivated, and clinically skilled Nurse Practitioner (NP) to help establish and grow this new service line.

This is an excellent opportunity for a provider who enjoys working with adult and geriatric populations and wants to play a key role in building a growing primary care program alongside an experienced Medical Director and interdisciplinary healthcare team.

Bilingual candidates are strongly preferred, as a significant portion of our patient population speaks Korean, Spanish, Vietnamese, Chinese, and other languages. Providers who can effectively communicate with diverse patient populations and families are highly valued.

Compensation

Compensation is based on experience, qualifications, certifications, demonstrated clinical competency, and overall fit for the role.

Candidates with significant experience in Primary Care, Geriatrics, Medicare/Medicaid populations, Chronic Care Management (CCM), Annual Wellness Visits (AWV), value-based care models, and bilingual or multilingual capabilities may qualify for compensation at the upper end of the pay range.

Responsibilities

• Perform comprehensive patient assessments and physical examinations.
• Diagnose and manage acute and chronic medical conditions.
• Develop, implement, and monitor individualized treatment plans.
• Prescribe medications and therapies in accordance with Virginia regulations.
• Conduct Annual Wellness Visits (AWV).
• Participate in Chronic Care Management (CCM) initiatives.
• Review laboratory and diagnostic results.
• Coordinate care with physicians, specialists, home health providers, hospice teams, caregivers, and family members.
• Maintain accurate and timely documentation within the electronic medical record system.
• Ensure compliance with Medicare, Medicaid, HIPAA, and applicable healthcare regulations.
• Collaborate closely with the Medical Director and interdisciplinary healthcare team.
• Assist in developing clinical workflows and quality improvement initiatives as the practice expands.
• Participate in patient and caregiver education regarding disease management, preventive care, medications, and treatment plans.
• Support care coordination efforts for patients receiving multiple healthcare services within the Evergreen network.

Qualifications

• Active Virginia Nurse Practitioner License.
• Board Certification as Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (AGNP), or related specialty.
• DEA Registration preferred or ability to obtain.
• Strong clinical assessment and diagnostic skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and collaboratively within a multidisciplinary team.
• Strong organizational and documentation skills.
• Commitment to providing compassionate, patient-centered care.

Preferred Qualifications

• Experience in Primary Care.
• Experience working with adult and geriatric populations.
• Experience with Medicare and Medicaid patients.
• Experience with Chronic Care Management (CCM).
• Experience conducting Annual Wellness Visits (AWV).
• Knowledge of value-based care programs.
• Experience with Electronic Medical Records (EMR) systems.
• Experience working in community-based healthcare settings.
• Bilingual or multilingual candidates are strongly preferred.
• Fluency in Korean, Spanish, Vietnamese, Chinese (Mandarin or Cantonese), or other languages commonly spoken within Northern Virginia communities is highly desirable.
• Experience serving culturally diverse patient populations.
