Job Description:

The Primary Care Physician will provide comprehensive, patient-centered primary care services to a diverse population, including Pediatrics, adults, and geriatrics. This role focuses on preventive health, chronic disease management, and acute care in an outpatient setting. Rancho Cucamonga offers a balanced lifestyle with proximity to the San Bernardino Mountains, Pacific Ocean (under 1 hour drive), and affordable housing in surrounding communities like Upland and Chino Hills. Expect a collaborative environment with 1:1 nursing support, minimal call, and emphasis on work-life balance.

Key Responsibilities

  • Conduct patient examinations, take detailed medical histories, and perform physical assessments for all age groups.
  • Diagnose and treat acute and chronic conditions, including hypertension, diabetes, respiratory infections, and preventive screenings (e.g., well-child visits, annual physicals, vaccinations).
  • Order, interpret, and follow up on diagnostic tests (e.g., labs, imaging, EKGs) and refer to specialists as needed.
  • Develop and implement personalized treatment plans, prescribe medications, and provide health education/counselling on lifestyle modifications.
  • Coordinate care with multidisciplinary teams, including nurse practitioners, physician assistants, and social workers, especially for Medicare-eligible seniors.
  • Maintain accurate electronic health records (EHR) using systems like Epic or Cerner; aim for 18-22 patients per day.
  • Participate in quality improvement initiatives, telehealth visits, and community health outreach.
  • Cover occasional extended hours or weekends if required (e.g., 1:10 call rotation).

Qualifications and Requirements

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited school.
  • Board certification in Family Medicine, Internal Medicine, or Pediatrics (required; recent graduates eligible if completing residency by 2025).
  • Active, unrestricted California medical license, DEA registration, and BLS/ACLS certification.
  • Minimum 3+ years of experience as a Primary Care Physician is required.
  • Strong interpersonal and communication skills; bilingual (Spanish/English) a plus due to diverse patient base.
  • Proficiency in EHR systems and evidence-based medicine; commitment to value-based care models (e.g., Medicare Advantage).
  • Ability to work independently and collaboratively in a fast-paced outpatient clinic.

Job Types: Full-time, Contract

Pay: $145.00 - $150.00 per hour
