About the Role

As a Primary Care Physician within our PACE program, you’ll work with a smaller, stable patient panel that allows you to build meaningful relationships and see the long-term impact of your care. This role is ideal for physicians who value holistic, person-centered healthcare and want the time and support to deliver it well.

What You’ll Do

As a Pod Leader, you will serve as a clinical cornerstone for an interdisciplinary care team. Your responsibilities include:

  • Conducting comprehensive, in-person evaluations and collaborating with the IDT to craft individualized care plans for each participant
  • Coordinating care transitions for participants requiring short-term, acute, or long-term placement
  • Delivering complete primary care without hospital rounds—expect to see roughly 8 patients per day
  • Participating in a rotating on-call system (phone-only), limited to a few evenings or weekends each month
  • Engaging in home visits during regular clinic hours (Monday–Friday, 8 AM–5 PM)

Who We’re Looking For

We welcome physicians specializing in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics who are passionate about senior care. Ideal candidates will have:

  • At least 1–2 years of clinical experience
  • Eligibility to treat Medicare and Medi-Cal participants
  • Board certification strongly preferred
  • A professional medical degree
  • No travel expectations for the role

Pay: $316,000.00 - $386,000.00 per year
