Primary Care Provider

Job Overview

AIM Wellness is currently seeking a compassionate and motivated Primary Care Provider to join our growing Rural Health Clinic team! We are open to APRNs, Family Nurse Practitioners, and Physicians (MD/DO) who are passionate about providing quality, patient-centered care to underserved communities.

This role offers the opportunity to make a meaningful impact through comprehensive outpatient care while working alongside a collaborative multidisciplinary team. Providers will deliver primary care services, manage acute and chronic conditions, promote preventative care, and support the overall health and wellness of patients across the lifespan.

Duties & Responsibilities

• Conduct patient assessments, physical examinations, and review medical histories
• Diagnose and treat acute and chronic medical conditions
• Develop and manage individualized treatment plans
• Prescribe medications and treatments in accordance with state and federal regulations
• Provide preventative care, wellness visits, and chronic disease management
• Maintain accurate and timely documentation within electronic health record (EHR) systems
• Collaborate with therapists, care coordinators, specialists, and support staff to provide comprehensive patient care
• Educate patients on medications, treatment plans, nutrition, and overall wellness
• Participate in telehealth services as needed to improve access to care
• Ensure compliance with HIPAA and clinic policies and procedures
• Support continuity of care through follow-up planning, referrals, and coordination of services

Qualifications

• Active Kentucky medical license or APRN license required
• Board certification as FNP, APRN, or Physician (MD/DO)
• DEA license or ability to obtain
• Strong communication and interpersonal skills
• Experience in primary care, family medicine, rural health, or outpatient settings preferred
• Experience with EHR/EMR systems preferred
• Ability to work independently while collaborating effectively within a team environment

Pay: $95,000.00 - $130,000.00 per year
