Primary Care Provider / Palliative Care Provider

As a Primary and Palliative Care Provider (Nurse Practitioner) at SWPCA you'll play a vital role in delivering life‑changing care by helping patients navigate a complex healthcare system with confidence and compassion, while upholding best‑practice standards.

Clinical Responsibilities

  • Evaluate, diagnose, and treat acute and routine medical conditions
  • Deliver care through in‑person visits, telephone, and telehealth
  • Manage patients across multiple settings (clinic, SNF, ALF, house calls)
  • Prescribe medications and treatment plans within scope of practice
  • Educate patients and families and evaluate outcomes of care
  • Maintain accurate, timely documentation in the medical record

Professional & Operational Responsibilities

  • Collaborate closely with the Clinic Manager and interdisciplinary care teams
  • Maintain compliance with state practice standards, HIPAA, OSHA, and safety protocols
  • Represent SWPCA Medical with professionalism, tact, and diplomacy
  • Participate in professional development, CME, and on‑call rotation as needed
  • Prioritize workload efficiently while maintaining high standards of care

Productivity Expectations

  • Visit volume is based on visit type and complexity
  • Higher‑acuity visits (MWV, TCM, etc.) may require fewer visits
  • All documentation must be completed within 72 hours of each visit

What We’re Looking For

  • Master’s degree from an accredited Nurse Practitioner program
  • Current state NP license and prescriptive authority in the State of Arizona
  • Ability to pass background check and drug screening
  • Valid driver’s license and reliable transportation
  • Comfortable traveling between care settings
  • Strong communication skills with patients, families, and healthcare partners
  • Solid understanding of federal and state healthcare regulations
