Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Duties
- Deliver comprehensive patient care by performing physical examinations, vital signs monitoring, and health assessments.
- Administer medications, injections, IV infusions, and treatments under physician supervision.
- Assist with medical procedures including catheterization, and specimen collection/processing.
- Maintain accurate medical documentation within electronic health record (EHR) systems, ensuring compliance with HIPAA regulations.
- Provide patient education on medication management, health coaching, dementia care strategies (including Alzheimer's and memory care), and discharge planning.
- Collaborate with multidisciplinary teams to coordinate case management for complex cases involving geriatrics, pediatric patients, or individuals with developmental disabilities.
- Support infection control protocols and aseptic techniques to ensure safety during all patient interactions.
Skills
- Proven experience working as an LPN in diverse settings such as hospitals, nursing homes, or home health, private duty nursing.
- Strong knowledge of medical terminology, physiology, anatomy, ICD-9/ICD-10 coding systems, and CPT coding for procedures.
- Skilled in using EMR/EHR systems for accurate documentation.
- Proficiency in performing IV insertions, phlebotomy, venipuncture, tube feeding management, and ventilator care.
- Excellent patient assessment skills with the ability to recognize acute pain issues and respond swiftly to urgent or emergency situations like airway management or trauma response.
- Experience caring for patients with infectious diseases or requiring hospice and palliative care services.
- Ability to work effectively with individuals of all ages including children (working with children), neonatal ventilator management or toddler care when necessary.
