PRN Home Health Aide

Duties

  • Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
  • Monitor and observe patient conditions, recording vital signs and reporting changes promptly
  • Support medication administration under supervision, ensuring proper documentation in electronic health record (EHR) systems
  • Provide specialized care for individuals with dementia, Alzheimer’s disease, or other memory impairments, including memory care techniques and Alzheimer’s care strategies
  • Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively
  • Assist with housekeeping tasks like cleaning, laundry, meal preparation, and maintaining a safe environment in accordance with HIPAA regulations
  • Document all patient interactions accurately in medical records and EHR systems while ensuring confidentiality and compliance with HIPAA standards

Experience

  • Prior experience working in assisted living facilities, nursing homes, hospice care, or home health environments is highly desirable
  • Demonstrated ability to perform patient observation and respond appropriately to changing conditions
  • Familiarity with medical terminology and basic medical documentation practices
  • Experience working with individuals with developmental disabilities or disabilities requiring specialized care such as dementia or Alzheimer’s disease
  • Certification in CPR is required; additional certifications like medication administration or catheter care are a plus
  • Knowledge of home & community care practices and long-term care experience enhances your ability to deliver comprehensive patient support
  • Strong communication skills paired with a compassionate approach to caregiving are essential for success in this role
