PRN Occupational Therapist (OT) – South Valley Post Acute and Rehab
Join our dynamic therapy team and make a direct impact on the independence and well-being of our residents. We are seeking a compassionate, licensed Occupational Therapist to provide person-centered care in our high-energy rehab department. This is a PRN position with potential opportunity to move to part-time or full-time. Some weekend shifts required.

Key Responsibilities
- Clinical Excellence: Conduct comprehensive patient evaluations within 48 hours of referral and design individualized treatment plans.
- Collaborative Care: Work closely with physicians, nursing staff, and families to coordinate discharge planning and maintenance programs.
- Interdisciplinary Participation: Active involvement in Patient Care Conferences, Utilization Reviews, and Functional Maintenance Programming.
- Equipment Coordination: Manage durable medical equipment (DME) and adaptive tools to ensure patient safety and independence in Activities of Daily Living (ADLs).
Qualifications
- Licensure: Active State Occupational Therapy License (Required).
- Clinical Knowledge: Strong understanding of occupational therapy standards of practice and the AOTA Code of Ethics.
- Documentation Skills: Proficiency in recording evaluations and progress notes (including HFCA/POC forms) with high attention to detail.
- Soft Skills: A positive attitude, a "team-player" mindset, and the ability to adapt to the fast-paced, unpredictable nature of a busy rehab environment.
Pay: $48.00 - $52.00 per hour
