Position Summary

The PRN Physician provides medical oversight, clinical consultation, and direct or indirect patient care for home health patients. This role collaborates closely with Nurse Practitioners, Registered Nurses, therapists, and social workers to ensure patients receive appropriate, timely, and effective medical management.


Key Responsibilities

  • Provide medical oversight and guidance to home health clinicians regarding patient treatment plans, medication management, and symptom evaluation.

  • Review, approve, and sign Plans of Care (POC), orders, recertifications, and physician-directed interventions as needed.

  • Offer timely medical consultations via phone, telehealth, or electronic communication.

  • Evaluate and provide recommendations for acute changes in condition, chronic disease exacerbations, and care escalation needs.

  • Conduct remote medical evaluations when appropriate to support patients unable to access in-person care.

  • Participate in interdisciplinary team (IDT) discussions, complex case reviews, or quality improvement meetings when available.

  • Ensure documentation of consults, orders, and medical oversight meets regulatory standards including Medicare/Medicaid requirements.

  • Complete timely documentation in the EMR (electronic medical record).

  • Maintain compliance with state and federal regulations governing home health services.

  • Provide coverage during high patient census, clinician PTO, or program expansion periods.

  • Support urgent medical inquiries and time-sensitive order reviews when available.

  • Assist with program development or quality initiatives as needed.


Qualifications

  • Doctor of Medicine (MD).

  • Active, unrestricted state medical license.

  • Board certification in Family Medicine, Internal Medicine, Geriatrics, or similar discipline (preferred).

  • Experience in home health, primary care, geriatrics, internal medicine, telemedicine, or community-based care strongly preferred.

  • Familiarity with Medicare home health regulations and care planning.

  • Strong communication skills, clinical judgment, and collaborative approach.

  • Ability to provide telehealth support with reliable internet and secure communication setup.

  • Experience with chronic disease management, transitional care, and home-based care models.

  • Comfort addressing acute medical issues remotely and coordinating escalations.

  • Compassionate, patient-centered decision-making.

  • Ability to work independently while supporting a multidisciplinary team.
