PRN PT

We have brought our therapy department in-house to provide an inviting work environment with reasonable productivity standards for their rehab staff and better care for their patients.

We are seeking a Physical Therapists to join our team PRN or Part-Time. The ideal candidate will work with patients to help them regain independence and improve their quality of life. This role involves collaborating with the facility staff to develop and implement individualized treatment plans, focusing on rehabilitation and functional improvement.

Responsibilities

• Assess patients' needs and develop tailored treatment plans based on their specific conditions.
• Provide therapeutic interventions to assist patients in achieving their rehabilitation goals.
• Conduct discharge planning to ensure a smooth transition for patients returning home or moving to the next level of care.
• Collaborate with multidisciplinary teams
• Educate patients and their families on techniques to improve daily living activities.
• Maintain accurate documentation of patient progress and treatment outcomes.

Requirements

• A degree in Physical Therapy from an accredited program.
• Valid California licensure or eligibility for licensure as a Physical Therapist.
• Experience working in SNF is a plus
• Excellent communication skills, both verbal and written, for effective interaction with patients, families, and healthcare teams.
• A commitment to ongoing professional development and staying current with best practices in physical therapy.
