Job Summary:

We are seeking a dedicated and compassionate Registered Nurse (RN) to join our team at Polaris Rehabilitation & Care Center. In this role, you will be responsible for providing high-quality care to residents, ensuring their health needs are met with professionalism and empathy. The ideal candidate will have strong clinical skills, a patient-centered approach to care, and a passion for working in a dynamic healthcare environment.

Key Responsibilities:

  • Resident Assessment & Care: Conduct comprehensive assessments of residents upon admission, throughout their stay, and when changes in condition occur. Monitor and evaluate residents' health status, identifying potential issues and implementing appropriate interventions.

  • Care Plan Development: Develop, implement, and update individualized care plans for residents. Work with the healthcare team to ensure that care plans reflect the residents’ needs, preferences, and medical conditions.

  • Medication Administration: Administer prescribed medications accurately and safely. Monitor for side effects and reactions, and document the administration in the residents' medical records.

  • Clinical Procedures & Treatments: Provide direct nursing care, including wound care, catheter care, IV therapy, vital sign monitoring, and other necessary medical treatments. Follow infection control protocols and ensure a safe and clean environment for residents.

  • Coordination of Care: Collaborate with physicians, other nurses, physical therapists, dietitians, and other healthcare professionals to coordinate care. Communicate effectively with families regarding residents' care, progress, and needs.

  • Resident and Family Education: Educate residents and their families about healthcare needs, treatments, medication management, and lifestyle choices that support well-being and recovery.

  • Supervision & Leadership: Supervise and mentor nursing assistants (CNAs) and other healthcare staff, ensuring they follow care protocols and maintain high standards of care. Provide training and guidance as necessary.

  • Documentation & Compliance: Maintain accurate and thorough documentation of nursing care provided, including assessments, treatments, medications, and progress. Ensure compliance with all regulatory requirements and facility policies.

  • Emergency Response & Critical Care: Respond quickly and effectively to emergency situations, providing necessary interventions, stabilization, and communication to physicians and emergency personnel.

Qualifications:

  • Registered Nurse (RN) with active licensure in Wyoming

  • Minimum of 2 years of nursing experience, preferably in [skilled nursing, long-term care, hospital setting, etc.].

  • Strong knowledge of nursing practices, clinical procedures, and healthcare regulations.

  • Excellent communication and interpersonal skills, with the ability to work well with residents, families, and the healthcare team.

  • Ability to assess, prioritize, and manage multiple patient needs effectively.

  • Compassionate, patient-centered approach to care with a strong focus on promoting the dignity and well-being of residents.

Physical Requirements:

  • Ability to lift, carry, and move patients, as necessary.

  • Ability to stand or walk for extended periods.

  • Ability to perform tasks that may require bending, reaching, or stretching.

  • Ability to work in environments with exposure to infectious materials, cleaning chemicals, and physical demands typical of a healthcare setting.
