<Duties>
- Perform detailed psychosocial assessments and develop individualized care plans tailored to each patient's needs and preferences.
- Provide mental health counseling, including individual and group therapy sessions, utilizing techniques such as cognitive behavioral therapy (CBT), motivational interviewing, and behavioral health interventions.
- Support patients and families through crisis intervention, grief counseling, and emotional support during difficult transitions.
- Collaborate with medical teams to coordinate hospice services, ensuring compliance with HIPAA regulations and accurate medical documentation within electronic medical record (EMR) systems.
- Assist with intake procedures, case management activities, and data collection for clinical research or quality improvement initiatives.
- Conduct diagnostic evaluations for mental health conditions such as PTSD or dementia-related behaviors; work with individuals with developmental disabilities or autism spectrum disorder as needed.
- Maintain thorough documentation of patient interactions, treatment plans, and progress notes in accordance with healthcare standards and ICD coding requirements.
<Requirements>
- Valid licensure as a Licensed Clinical Social Worker (LCSW) or equivalent in the state of practice.
- Proven experience in hospice care, hospital settings, or home & community-based services; inpatient or nursing home experience preferred.
- Strong knowledge of medical terminology, ICD-10/ICD-9 coding systems, CPT coding for psychotherapy services, and HIPAA compliance.
- Demonstrated skills in child & family counseling, crisis management, behavioral therapy, and working with individuals with disabilities or dementia care needs.
- Proficiency in EMR systems and data collection tools; experience with post-acute care documentation is a plus.
- Excellent communication skills for intake assessments, diagnostic evaluations, and collaboration within multidisciplinary teams.
