Join our compassionate team as a PRN Hospice Social Worker, where you will play a vital role in providing emotional support, counseling, and comprehensive case management to patients and their families facing end-of-life care. Duties
- Conduct thorough patient assessments to determine psychosocial needs, including mental health status, family dynamics, and community resources.
- Provide individual, family, and group counseling sessions utilizing techniques such as motivational interviewing, cognitive behavioral therapy, and behavioral therapy to support emotional well-being.
- Coordinate with healthcare providers to develop personalized care plans that incorporate hospice care principles, mental health counseling, and addiction counseling when appropriate.
- Manage intake processes efficiently while ensuring compliance with HIPAA regulations for medical documentation and records management.
- Facilitate crisis intervention and crisis management strategies to address urgent emotional or behavioral health issues.
- Collect data for clinical research projects related to hospice care outcomes and behavioral health interventions.
- Assist with post-acute care planning, including working with patients with dementia, Alzheimer's disease, PTSD, autism spectrum disorders, or developmental disabilities.
- Maintain accurate medical records using EMR (Electronic Medical Records) systems and ensure proper documentation of all patient interactions in accordance with medical terminology standards such as ICD-10/ICD-9 coding.
Skills
- Experience in family counseling, mental health counseling, and clinical social work within hospice or palliative settings.
- Strong knowledge of medical terminology, CPT coding, ICD coding systems, and HIPAA compliance for confidential medical documentation.
- Proven ability to perform diagnostic evaluations and case management for diverse populations including geriatrics, pediatrics, individuals with disabilities or developmental disorders.
- Skilled in crisis intervention techniques and trauma-informed care approaches such as PTSD care or Alzheimer's/dementia care strategies.
- Familiarity with EMR systems for data collection and documentation; experience working within inpatient, outpatient, nursing home, assisted living or home & community care environments.
- Ability to work effectively with interdisciplinary teams across hospital experience or outpatient clinics; experience in research is a plus.
- Excellent communication skills for conducting intake interviews, individual/group therapy sessions, and family meetings; adept at motivational interviewing and behavioral health counseling.
- Knowledge of working with individuals with autism spectrum disorder or other developmental disabilities; experience in special education or pediatric settings is advantageous.
