About Central Texas Table of Grace
We know it takes a village to raise a child. Foster youth don’t have that village, but Central Texas Table of Grace creates it. Founded by former foster youth Stacy Johnson, we provide comprehensive support for foster youth from early childhood through young adulthood. Our Supervised Independent Living (SIL) program serves young adults ages 18 to 21 who have aged out of foster care, providing subsidized housing, life skills programming, educational support, career development, and ongoing mentorship to help them successfully transition to independence.
Position Summary
The SIL Program Director and Case Manager leads our Supervised Independent Living program and provides direct case management services to the young adults we serve. As the sole staff member dedicated to this program, this role carries full responsibility for program operations, compliance, and participant outcomes. The SIL Program Director and Case Manager serves on the CTTG leadership team, collaborates with DFPS and/or SSCC caseworkers, PAL staff, and community partners, and ensures our young adults receive consistent, high-quality support as they transition to adulthood.
Key Responsibilities
- Lead all day-to-day operations of the SIL program, including program planning, implementation, and continuous improvement
- Provide direct case management services to young adults ages 18 to 21 in the SIL program, including regular check-ins, goal setting, and progress monitoring
- Develop and maintain individualized service plans in collaboration with each young adult and their DFPS or SSCC caseworker
- Serve on the CTTG leadership team, contributing to organizational strategy and cross-program coordination
- Ensure full compliance with all DFPS contract requirements, reporting obligations, and organizational standards
- Support young adults in building life skills including budgeting, cooking, time management, healthcare navigation, and employment readiness
- Coordinate with DFPS and/or SSCC caseworkers, PAL staff, STAR Health providers, educational institutions, employers, workforce development boards, and community resources to ensure comprehensive support
- Design and facilitate life skills workshops and group programming
- Manage program data, outcomes tracking, and reporting to support organizational decision-making and funder requirements
- Monitor apartment conditions and manage housing-related concerns, including landlord and property management relationships
- Respond to crisis situations and connect young adults to appropriate emergency resources
- Support young adults in developing and maintaining healthy relationships and community connections
- Develop and maintain community partnerships that expand resources available to SIL participants
- Manage the SIL program budget in coordination with organizational leadership
- Maintain the organization and cleanliness of the SIL program office building, including receiving and organizing donated supplies and materials, developing and maintaining systems to keep the building organized, tidy, and supplies and donations systematized, managing trash and recycling pickup, and working with volunteers who support yard maintenance and general upkeep of the facility
- Transport or coordinate transportation for young adults to appointments, interviews, and other essential activities as needed
- Represent the SIL program in meetings with DFPS, SSCCs, PAL staff, and other stakeholders

Required Qualifications
- Bachelor's degree in social work, psychology, human services, or a closely related field
- Minimum of 1 year of experience in case management, social services, or direct service with young adults or transitional-age youth
- Valid Texas driver's license and reliable transportation
- Must pass DFPS background check and meet all DFPS and HHSC caregiver qualification requirements
- Strong written and verbal communication skills
- Demonstrated ability to manage program operations, including compliance, documentation, and outcomes reporting
- Ability to work some evenings and weekends as program needs require
- Proficiency with basic computer applications and case management documentation systems
Preferred Qualifications
- 2+ years of experience in case management, social services, or direct service with young adults or transitional-age youth
- Program leadership or supervisory experience
- Licensed Bachelor Social Worker (LBSW) or Licensed Master Social Worker (LMSW)
- Experience working specifically with foster youth, youth aging out of care, or transitional-age youth populations
- Familiarity with DFPS systems, SSCC/Community-Based Care, PAL services, Extended Foster Care, and SIL program requirements
- Experience managing program budgets and funder reporting
- Trauma-informed care training or certification
- Bilingual (English/Spanish)
