ABOUT THE C.H.A.N.C.E. PROJECT
The C.H.A.N.C.E. Project is an innovative 24-month health equity pilot program led by The Literacy Center in partnership with the Attleboro Norton YMCA, Attleboro Council on Aging, Attleboro Public Schools, Attleboro Area Interfaith Collaborative, and The Arc of Bristol County. Funded by Sturdy Health, the project aims to train and deploy a 10-person cohort of new Community Health Workers (CHWs) and 15–20 volunteer Peer Health Navigators (PHNs) across the greater Attleboro region to connect vulnerable community members — including immigrants, older adults, people with disabilities, and individuals experiencing poverty — to health and social services through a "No Wrong Door" model. The aim of this project is to prove that when a community has enough healthcare supports, and the right people in place, all members of the community can improve their health outcomes and thrive.
POSITION SUMMARY
The position of Program Manager is an exciting opportunity for someone who enjoys building projects from the ground up and working collaboratively with cross-sector experts, with a spirit of creativity, flexibility and ambitiousness. This senior role oversees day-to-day project operations, coordinates the community health worker training program and deployment of workers into the community, manages multi-partner collaboration, and leads all data collection and evaluation efforts. The Program Manager ensures the project's equity-centered values are embedded in every aspect of service delivery and that outcomes are achieved with fidelity, cultural responsiveness, and community voice at the center.
