ABOUT THE C.H.A.N.C.E. PROJECT

The C.H.A.N.C.E. Project is an innovative 24-month health equity pilot program led by The Literacy Center in partnership with the Attleboro Norton YMCA, Attleboro Council on Aging, Attleboro Public Schools, Attleboro Area Interfaith Collaborative, and The Arc of Bristol County. Funded by Sturdy Health, the project aims to train and deploy a 10-person cohort of new Community Health Workers (CHWs) and 15–20 volunteer Peer Health Navigators (PHNs) across the greater Attleboro region to connect vulnerable community members — including immigrants, older adults, people with disabilities, and individuals experiencing poverty — to health and social services through a "No Wrong Door" model. The aim of this project is to prove that when a community has enough healthcare supports, and the right people in place, all members of the community can improve their health outcomes and thrive.

POSITION SUMMARY

The position of Program Manager is an exciting opportunity for someone who enjoys building projects from the ground up and working collaboratively with cross-sector experts, with a spirit of creativity, flexibility and ambitiousness. This senior role oversees day-to-day project operations, coordinates the community health worker training program and deployment of workers into the community, manages multi-partner collaboration, and leads all data collection and evaluation efforts. The Program Manager ensures the project's equity-centered values are embedded in every aspect of service delivery and that outcomes are achieved with fidelity, cultural responsiveness, and community voice at the center.

KEY RESPONSIBILITIES

Program Operations & Leadership

• Oversee all day-to-day operations of the C.H.A.N.C.E. Project across 10–15 partner sites
• Serve as primary point of coordination between The Literacy Center, all lead partners, and host site agencies
• Manage project timelines, milestones, and phase transitions (foundation → training → scale → sustainability)
• Supervise and provide reflective supervision to health workers and the Resource Navigator

CHW & PHN Workforce Development

• Coordinate recruitment, screening, and enrollment of 15–20 career-track CHW trainees across two cohorts
• Oversee CHW training partnerships with contracted college
• Support fieldwork placements and ensure culturally responsive mentorship for all trainees
• Collaborate with The Literacy Center's Career Advisors to support CHW employment transitions

Data, Evaluation & Reporting

• Lead equity-centered data collection using various standardized screening tools and self-efficacy surveys
• Maintain HIPAA-compliant outcomes database
• Prepare and submit quarterly funder reports, monthly partner data presentations, and a final comprehensive evaluation report
• Convene and facilitate quarterly stakeholder meetings and the Annual Community Wellness Forum

Community Outreach & Partnerships

• Build and maintain trusted relationships across community partners, host sites, and health agencies including Sturdy Hospital and Manet Health
• Coordinate with CHWs and PHNs to implement multilingual outreach strategies across Attleboro communities
• Support the Resource Navigator in developing the unified cross-agency community resource platform

REQUIRED QUALIFICATIONS

• Master's degree in Social Work (MSW), Public Health (MPH), or closely related field
• Minimum five (5) years of experience in program management, community health, or human services — preferably in a nonprofit or public health setting
• Demonstrated experience managing multi-partner collaborations or community coalitions
• Strong data management and reporting skills; experience with health needs screening tools a plus
• Deep commitment to health equity, racial justice, disability inclusion, and serving marginalized communities
• Excellent written and verbal communication skills; ability to present to diverse audiences
• Experience with HIPAA-compliant data systems and grant reporting

PREFERRED QUALIFICATIONS

• Bilingual in Spanish, Haitian Creole, Portuguese, or another language greatly preferred
• Experience with CHW training programs or workforce development initiatives
• Familiarity with PRAPARE, Lubben Scale, or other standardized social determinants screening tools
• Knowledge of Massachusetts social service landscape, health equity resources, or EOHHS systems
