Program Specialist / Program Manager - ODP IDD Home Care

Join our dedicated team as a Program Specialist / Program Manager for ODP IDD Home Care, where you will play a vital role in coordinating and overseeing home and community-based services for individuals with intellectual and developmental disabilities (IDD). This dynamic position offers an opportunity to make a meaningful difference by supporting individuals with diverse needs, including autism, behavioral health challenges, and disabilities. You will lead efforts to ensure personalized care plans are implemented effectively, promote independence, and enhance quality of life through compassionate support and meticulous program management. Your energetic approach will help empower individuals to thrive in their communities while fostering collaboration among families, caregivers, and service providers.

Responsibilities

  • Develop, implement, and monitor individualized care plans tailored to each person’s unique needs, including assistance with activities of daily living (ADLs) such as bathing, dressing, and medication management
  • Coordinate services across multiple providers to ensure seamless delivery of home and community-based supports
  • Conduct regular assessments and data collection to evaluate progress, adjust care strategies, and ensure compliance with program standards
  • Provide direct support and training to caregivers and families on behavioral health strategies, behavior management techniques, communication skills—including American Sign Language—and other caregiving best practices
  • Assist individuals with social integration by facilitating community outings, childcare activities, and educational opportunities while promoting independence
  • Maintain detailed documentation related to service delivery, medication administration records, incident reports, and progress notes in accordance with regulatory requirements
  • Drive program success by collaborating with interdisciplinary teams—including social workers, special educators, and behavioral health specialists—to optimize outcomes

Requirements

  • Proven experience working with individuals with developmental disabilities, autism spectrum disorder, or other disabilities in home or community settings
  • Strong understanding of special education principles and behavioral health interventions
  • Excellent communication skills to effectively engage with clients, families, caregivers, and team members
  • Ability to assist with ADLs and provide compassionate caregiving in various environments
  • Experience in data collection, documentation accuracy, and maintaining compliance standards
  • Valid driver’s license with reliable transportation for community outings and home visits
  • Knowledge of medication administration procedures and safety protocols is preferred
  • Bilingual abilities such as American Sign Language or additional languages are a plus

Pay: $25.08 - $30.21 per hour
