The Program Supervisor of Case Management & Care (OHFH) is a pivotal leadership role dedicated to supporting individuals with Intellectual and Developmental Disabilities (IDD) living independently. This role will oversee the Own Home / Family Home (OHFH) program, ensuring clients receive high-quality, person-centered care.
Reporting directly to the Long Term Care Program Manager, the Program Supervisor will provide comprehensive case management for our OHFH clients, lead and mentor a team of CFC Attendants, and ensure seamless daily operations. This position requires a unique blend of administrative oversight, staff coaching, and a hands-on commitment to care.

- Compensation: $21/hr
- Schedule: Tuesday – Friday, 10:00 AM – 8:00 PM (40 hours/week, 4-day work week)

Essential Responsibilities
Case Management & Client Advocacy
- Serve as the primary case manager for OHFH clients, escalating complex or high-acuity cases to the LTC Program Manager as needed.
- Conduct regular check-ins and assessments to ensure clients' health, safety, and satisfaction.
- Act as the primary liaison between clients, families, medical professionals, and external service coordinators.
Staff Supervision & Training
- Directly supervise, mentor, and evaluate a team of CFC Attendants.
- Provide ongoing, on-the-job coaching and formal training to ensure staff adhere to best practices in IDD care and compliance regulations.
- Foster a supportive, collaborative, and accountable team culture.
Operations, Staffing & Schedule Management
- Mitigate staffing gaps proactively by finding coverage or adjusting schedules.
- Act as the ultimate safety net for the program. If a shift cannot be filled, you will step in and function as the CFC Attendant to ensure client safety and continuity of care.
Compliance & HHSC Standards
- Ensure all documentation submitted by CFC Attendants are accurate, timely, and compliant with state and agency standards.
- Participate in incident reporting and investigations as required.
Qualifications & Skills
Required:
- Minimum of 2 years of experience working with individuals with IDD, with a strong understanding of independent living supports
- Proven experience in a supervisory role
- Exceptional interpersonal, written, and verbal communication skills
- Technological literacy
- Valid driver's license, reliable vehicle, and a willingness to transport clients
- Ability to pass a background check
Preferred:
- Associate's degree in social work, psychology, human services or a related field
- Prior experience with medicaid waiver programs
Pay: $21.00 per hour
