Project Manager - State Gov Grants - open to travel

Role description:

South Dakota Department of Social Services (DSS) is seeking a qualified individual(s) for program management of DSS initiatives being funded under the Rural Health Transformation (RHT) Grant. DSS has three specific initiatives in the RHT grant titled Medicaid Primary Accountable Care Transformation (PACT), Medicaid Rural Health Access and Quality Grants, and Behavioral Health. There is a need for project management services a) to ensure comprehensive coordination across these distinct yet related projects b) to assist with reporting outcome measures, c) to achieve the strategic alignment sought by South Dakota, and d) to manage interconnected (inter-Departmental, inter-divisional, and cross-program) dependencies, resources, and risks for this large-scale transformation.

No sponsorship is available. Candidates must be willing to travel quarterly to SD, the primary work will be done remotely.

Job Description, activities include but are not limited to:

  • Serve as overall project management support across multiple rural health transformation initiatives,
  • Develop and maintain integrated workplans, timelines, milestones, and deliverables
  • Hold key personnel accountable for task completion
  • Project support including research, meeting facilitation and logistics, project task follow up
  • Schedule and facilitate regular meetings, stakeholder check-ins, and progress reviews
  • Track risks, dependencies, and mitigation strategies across initiatives
  • Support compliance with federal grant requirements, reporting deadlines, and allowable activities.
  • Prepare status reports, progress reports, briefings, and documentation for state leadership and CMS as needed
  • Assist DSS staff with federal evaluation grant evaluation activities
  • Support design, refinement, and implementation of Medicaid Primary Accountable Care Transformation (PACT) model
  • Support development of policies, guidance, technical assistance, and operational workflows
  • Assist in creation of communications including written documents and presentations as needed
  • Assist DSS staff in creation, evaluation, and award of RFPs as well as contract renewals, amendments, and scope changes
  • Assist DSS staff with monitoring and oversight of subrecipients of federal grant funding.
  • Assist DSS staff with addressing and remedying issues of non-compliance or underperformance by subrecipients of federal grants
  • Monitor budgets and expenditures in coordination with finance staff
  • Track vendor deliverables
  • Coordinate with other project management staff as needed including any internal or vendor support
  • Consultation Process: coordinate provider outreach, education, and technical assistance related to PACT
  • Coordinate with other state agencies including Department of Health and Governor’s Office as needed
  • Assist in creating stakeholder engagement group for PACT initiative including member recruitment as well as assistance in supporting and facilitating this group and their work
  • Assist in creating payer engagement framework including assistance in supporting group with meetings and deliverables
  • Assist DSS staff in creating case management framework including integration of population health and case management tools
  • Assist DSS staff with planning and execution of annual Case Management, Social Worker, and Community Health Worker Medicaid Workshop in support of PACT and RHT goals
  • Ongoing/Post-Launch
  • Support development and monitoring of quality and cost measures for PACT
  • Assist with analysis of model performance and help trouble-shoot implementation challenges
  • Coordinate with data vendors as needed to be able to provide reporting on PACT’s goals and deliverables
  • Creating a monthly status report associated with any work accomplished timelines related to this work order.
  • For the Medicaid Rural Health and Access Grant initiative, key responsibilities:
  • Grand administration
  • Develop and implement systems for tracking grant applications, awards, disbursements, and reporting.
  • Provide technical assistance to grantees on application processes, eligibility, and compliance requirements.
  • Maintain accurate and up-to-date records of all grant-related activities.
  • Program Oversight and Monitoring
  • Monitor grantee performance and ensure alignment with program goals and funding requirements.
  • Conduct regular reviews of grantee progress reports, financial statements, and deliverables.
  • Identify and address issues of non-compliance or underperformance.
  • Post Award Activities
  • Provide technical assistance to grantees including but not limited to regular TA office hours, meetings, webinars, and other educational outreach.
  • Serve as a liaison between the funding agency and grantees.
  • Develop and distribute program updates, guidance documents, and best practices.
  • Data Collection and Reporting
  • Collect, analyze, and report on program metrics and outcomes.
  • Prepare quarterly and annual reports for internal and external stakeholders.
  • Support evaluation efforts to assess program impact.
  • Regulatory and Fiscal Compliance
  • Ensure all activities comply with applicable federal and state regulations.
  • Provide fiscal oversight, including expenditure reviews, and audit preparation.
  • Support risk management and internal control processes.

Experience:

  • At least 3 years of project management experience
  • Strong understanding of formal project management methodologies
  • Project management/change management certification preferred (examples include Certified Change Management Professional (CCMP) Project Management Professional (PMP))
  • Experience in large-scale healthcare transformation projects
  • Experience with Medicaid with additional preference for experience with alternative payment models.
  • Experience with coordinating multi-vendor initiatives preferred
  • Experience with contracting and the RFP process.
  • Experience with CMS grant reporting and accountability expectations
  • Knowledge of federal behavioral health requirements and best practices
  • Strong facilitation and documentation skills

Education and Certifications

  • Bachelor’s degree in related field is required
