PRP case manager

Job Overview
We are seeking a dedicated and energetic PRP (Psychiatric Rehabilitation Program) Case Manager to join our dynamic Behavioral Health Foundation. In this role, you will coordinate comprehensive patient care plans, facilitate seamless discharge processes, and ensure optimal post-acute and outpatient services. Your expertise will help bridge the gap between Substance Misue Treatment and community resources, entitlements, and financial empowerment to achieve better outcomes. Responsibilities

• Collaborate with multidisciplinary teams such as social workers, behavioral health specialists, addiction counselors, and medical staff to develop personalized care plans.
• Manage utilization review processes within managed care settings, ensuring compliance with HIPAA regulations and medical terminology standards.
• Facilitate intake procedures for new patients, ensuring accurate documentation and efficient processing of medical records.
• Must have knowledge of computer use.
• Support patient service initiatives by providing education on treatment options, discharge instructions, and community resources to promote self independence. recovery and ongoing health management.

Qualifications

• Familiarity with managed care processes, utilization management, inpatient/outpatient settings, and post-acute care coordination.
• Excellent communication skills with the ability to collaborate effectively across multidisciplinary teams.

Pay: From $24.17 per hour
