About us

ReMind Health is a psychiatric provider group focused on geriatric patients needing psychiatry located in nursing homes, assisted living, and skilled nursing facilities. Our aim is to provide mental health services to a segment of the community that is often “forgotten” during their rehabilitation and journey back to baseline health. Many of these patients will be discharging to their homes within several weeks. These patients require mental health support during these often difficult transitional times adapting to their new medical diagnosis.

GOALS:

● Support the patient while they transition to their new baseline

● Provide accurate and timely charting to the facility

● Recommend medication changes to attending provider

● Be a good partner with the hosting facility

● Facility based work is very flexible

● Make your own hours

● Low level of psychiatric acuity in patient population

About you:

● PMHNP licensed (unencumbered) in the state of Utah

● Computer literate

● Comfortable in nursing homes

● Compassionate

● Willing to learn

Pay: $95,000.00 - $120,000.00 per year
