Psychiatric Mental Health Nurse Practitioner

The Saratoga ACT team (Assertive Community Treatment team) is a multidisciplinary team including clinicians, nurses, prescriber(s), and case managers. The ACT team provides intensive, community-based mental health support to people with severe and persistent mental illness. Services are delivered in the person’s home or community, available 24/7, and focus on helping individuals stay stable, avoid hospitalization, and build skills for independent living.

The ACT Team in Saratoga is seeking a contracted Psychiatric Nurse Practitioner (27.5 hours per week). This requires hours Monday through Friday with flexibility in scheduling development.The Psychiatric Provider is a key member of the multidisciplinary ACT team, responsible for delivering psychiatric assessment, medication management, and clinical oversight to individuals with severe and persistent mental illness. The provider works primarily in community settings—homes, shelters, and other natural environments—to ensure flexible, person-centered care that promotes stability, recovery, and community integration.

Essential Duties & Responsibilities:

  • Conduct comprehensive psychiatric evaluations, diagnostic assessments, and ongoing medication management.
  • Develop, implement, and update individualized treatment plans in collaboration with the ACT team.
  • Provide education to clients and families on diagnoses, medications, side effects, and symptom management.
  • Participate in daily team meetings to review client status, guide treatment interventions, and ensure coordinated care.
  • Maintain availability for crisis intervention and support as part of the ACT team’s 24/7 coverage model (may include on-call rotation).
  • Collaborate closely with nurses, clinicians, case managers, peers, and community partners to promote holistic, recovery-oriented care.
  • Document all services in accordance with agency policies, regulatory standards, and billing requirements.
  • Monitor client progress, identify risks, and adjust treatment plans as clinically indicated.
  • Promote integration of physical and mental health care by coordinating with primary care and specialty providers.

Qualifications:

  • Licensed Psychiatrist (MD/DO), Psychiatric Nurse Practitioner (PMHNP), or other qualified psychiatric prescriber eligible to practice in the state.
  • Valid NYS Driver's License
  • Experience working with individuals with severe mental illness, co-occurring disorders, and high service needs preferred.
  • Familiarity with the ACT model or intensive community-based treatment strongly preferred.
  • Strong clinical decision-making, communication, and crisis-management skills.
  • Ability to work effectively in the field, travel within the community, and engage clients in nontraditional settings.
  • Commitment to recovery-oriented, person-centered, and trauma-informed practice.

Work Environment:

This position involves significant work in the community and client homes. Requires flexibility, collaboration, and comfort working with diverse populations experiencing complex clinical and social needs.
