This is not a traditional high-volume clinic role.

Position Overview

The PMHNP will provide behavioral health support services within senior care facilities, including:

✔ Behavioral and psychiatric assessments

✔ Medication review and management

✔ Resident stabilization support

✔ Collaboration with facility staff

✔ Behavioral recommendations and care coordination

✔ Ongoing follow-up care

✔ Family communication support when appropriate

Ideal Candidate Profile

We are specifically looking for providers who are:

✔ Experienced with geriatric populations

✔ Familiar with assisted living, memory care, or SNF environments

✔ Comfortable working independently

✔ Reliable and responsive

✔ Interested in building meaningful facility relationships rather than high patient volume

Requirements

✔ Active California PMHNP license

✔ DEA registration preferred

✔ Active malpractice insurance

✔ Experience in geriatric psychiatry, behavioral health, senior care, or long-term care settings preferred

✔ Ability to travel locally within assigned territories
