As a Care Coordinator with Adult & Child Health / Brightli, you will play a key role in supporting individuals receiving behavioral health and substance use treatment. You’ll work closely with clients, families, clinicians, and community partners to coordinate services, remove barriers to care, and help clients achieve meaningful progress in their recovery and overall wellbeing.

What You’ll Do

As a Care Coordinator, you will serve as a central point of support for clients receiving outpatient behavioral health services.

Key responsibilities include:

Care Coordination & Client Support

  • Review initial assessments and identify client needs related to healthcare, housing, employment, education, social support, and legal systems

  • Coordinate outpatient behavioral health and substance use services

  • Assist clients and families in accessing community resources and supportive services

  • Advocate for clients and help remove barriers to care

Collaboration & Communication

  • Maintain regular communication with referral sources, primary care providers, and treatment teams

  • Participate in treatment team meetings and clinical staffings

  • Coordinate referrals to outside providers when additional services are needed

  • Represent the organization professionally within the community

Treatment Planning & Documentation

  • Support the development and ongoing updates of individualized treatment plans

  • Ensure accurate and timely documentation of all services provided

  • Address and correct any chart documentation deficiencies

  • Assist clients and families with transition and discharge planning

Crisis & Client Support

  • Provide crisis intervention and support when needed

  • Help coordinate transportation and scheduling for treatment services

  • Ensure clients receive appropriate referrals for medical or additional support services

Knowledge, Skills, and Abilities

Successful candidates will demonstrate:

  • Knowledge of case management and rehabilitation practices in behavioral health or substance use treatment

  • Understanding of community resources and support systems

  • Ability to collaborate with multidisciplinary teams

  • Strong communication, organization, and documentation skills

  • Ability to advocate effectively for client needs

Education & Experience

Preferred qualifications include:

  • Bachelor’s degree in a human services field, such as:

    • Social Work

    • Psychology

    • Counseling

    • Criminal Justice

    • Human Development and Family Studies

    • Nursing or Education

Experience working with behavioral health, substance use treatment, or community-based services is strongly preferred.

Additional Employment Requirements

  • Successful completion of background screening including criminal, driving, abuse/neglect, and fingerprint checks

  • Valid driver’s license, acceptable driving record, and current auto insurance

  • Completion of required onboarding and training programs

  • CPR and First Aid certification within three months of hire

  • Completion of behavioral management training within 30 days of hire

  • Ongoing professional development training requirements

Physical Requirements

This position primarily involves sedentary work, including sitting for extended periods while completing documentation and coordinating services. Occasional walking or standing may be required. Duties may include repetitive movements such as typing and writing during the workday.
