Key Responsibilities
- Assess residents’ social, emotional, and psychological needs in order to develop and implement individualized care plans.
- Provide counseling and therapeutic support to residents and their family members, addressing concerns such as adjustment to nursing home placement, grief and loss, and family dynamics.
- Coordinate and facilitate social, emotional, and therapeutic support groups for residents.
- Advocate for residents’ psychosocial needs and ensure their rights, dignity, and well-being are upheld.
- Assist with discharge planning and support smooth transitions from the facility to other care settings or home environments.
- Collaborate closely with the interdisciplinary care team to ensure comprehensive, resident-centered care delivery.
- Maintain accurate, timely, and confidential documentation in compliance with state and federal regulations.
- Provide crisis intervention and emotional support during challenging psychological or emotional situations.
- Support and assist the facility in maintaining compliance with regulatory and accreditation requirements related to psychological and social work services.
Pay: $85,000.00 - $135,000.00 per year
