PT Physical Therapist per diem/contract/part time

Physical Therapist - contract, part-time. Home health. Facility in Palmetto 2 half days.
Flex days/hours: you set your schedule. Supplement your income with a few visits per week.
We are looking for Physical Therapist per diem/contract evaluations and visits (not Medicare).
NON-OASIS PT Evaluation $75 and subsequent PT Visit $70 with your patient.
Licensed by AHCA, accredited by CHAP to provide skilled nursing and therapy services at facilities and home care.
