We are seeking compassionate and skilled clinicians—including Physical Therapists (PT), Physical Therapist Assistants (PTA), Occupational Therapists (OT), Certified Occupational Therapy Assistants (COTA), and Speech-Language Pathologists (SLP)—to provide high-quality home health services. Clinicians will deliver patient-centered care in the comfort of patients’ homes, helping them regain independence and improve overall quality of life.
Key Responsibilities
- Provide skilled therapy services in accordance with physician orders and plan of care
- Evaluate patients (PT, OT, SLP) and develop individualized treatment plans
- Implement therapeutic interventions to improve mobility, function, ADLs, communication, and swallowing
- Educate patients and caregivers on treatment techniques, safety, and home programs
- Monitor patient progress and adjust care plans as needed
- Collaborate with interdisciplinary healthcare team members
- Maintain timely, accurate, and compliant documentation
- Ensure adherence to home health regulations and agency policies
QualificationsPT / OT / SLP
- Graduate of an accredited program (DPT, OTD/MOT, or Master’s in Speech-Language Pathology)
- Current Florida state licensure
- Minimum 1 year of clinical experience (home health preferred (will train)
PTA / COTA
- Graduate of an accredited assistant program
- Current Florida state licensure
- Experience in home health preferred (will train)

- Valid driver’s license and reliable transportation
- CPR certification
- Strong communication and organizational skills
- Ability to work independently

Projected Total Compensation: $60,000.00 - $160,000.00 per year
