PT | PTA | OT | COTA | SLP-Home Health: Martin County, FL

We are seeking compassionate and skilled clinicians—including Physical Therapists (PT), Physical Therapist Assistants (PTA), Occupational Therapists (OT), Certified Occupational Therapy Assistants (COTA), and Speech-Language Pathologists (SLP)—to provide high-quality home health services. Clinicians will deliver patient-centered care in the comfort of patients’ homes, helping them regain independence and improve overall quality of life.

Key Responsibilities

• Provide skilled therapy services in accordance with physician orders and plan of care
• Evaluate patients (PT, OT, SLP) and develop individualized treatment plans
• Implement therapeutic interventions to improve mobility, function, ADLs, communication, and swallowing
• Educate patients and caregivers on treatment techniques, safety, and home programs
• Monitor patient progress and adjust care plans as needed
• Collaborate with interdisciplinary healthcare team members
• Maintain timely, accurate, and compliant documentation
• Ensure adherence to home health regulations and agency policies

QualificationsPT / OT / SLP

• Graduate of an accredited program (DPT, OTD/MOT, or Master’s in Speech-Language Pathology)
• Current Florida state licensure
• Minimum 1 year of clinical experience (home health preferred (will train)

PTA / COTA

• Graduate of an accredited assistant program
• Current Florida state licensure
• Experience in home health preferred (will train)

All Positions

• Valid driver’s license and reliable transportation
• CPR certification
• Strong communication and organizational skills
• Ability to work independently

Projected Total Compensation: $60,000.00 - $160,000.00 per year
