Job Overview
We are seeking a dedicated and compassionate Physical Therapist Assistant to join our healthcare team. The ideal candidate will work under the supervision of a licensed Physical Therapist to assist in the implementation of patient care plans. This role is essential in providing high-quality rehabilitation services to patients across various settings, including skilled nursing facilities, clinics, and home health environments.

Duties

• Assist Physical Therapists in developing and implementing individualized care plans for patients.
• Provide direct patient care, including therapeutic exercises and modalities.
• Monitor patient progress and report changes to the supervising Physical Therapist.
• Educate patients and their families on treatment plans and home exercise programs.
• Participate in discharge planning to ensure a smooth transition for patients.
• Maintain accurate documentation of patient treatments and progress notes.
• Collaborate with interdisciplinary teams to optimize patient outcomes.
• Support the physical therapy team in maintaining a clean and organized work environment.

Experience

• Knowledge of human physiology and anatomy is essential for effective patient care.
• Experience working with diverse populations, including geriatrics, pediatrics, and acute care patients.
• Familiarity with discharge planning processes and skilled nursing facility protocols is advantageous.
• Previous experience in home health or clinic settings is preferred but not required.
• Strong communication skills to effectively interact with patients, families, and healthcare professionals.
• A commitment to providing compassionate care and supporting patients in their rehabilitation journey.
