Position Summary
The CMH Public Health Nurse maintains and manages a caseload of clients enrolled in the Complex Medical Help (CMH) Program. This role focuses on care coordination, home visitation, health education, advocacy, and connecting families with healthcare and community resources to improve health outcomes and quality of life.
Essential FunctionsCase Management & Client Support
- Maintain and manage a caseload of CMH clients
- Conduct in-person home visits, field visits, telehealth, and phone encounters
- Assess client health needs, support systems, and living environments
- Collaborate with families to develop individualized care plans and health goals
- Coordinate referrals and connect families to medical and community services
- Provide education, guidance, and ongoing support to families
- Continuously reassess client needs to ensure services remain appropriate and effective
- Maintain knowledge of local, regional, and state health and socioeconomic resources
- Coordinate care with physicians, hospitals, specialty clinics, schools, agencies, and partner organizations
- Respond to inquiries from healthcare professionals, families, and community partners
Documentation & Compliance
- Maintain accurate and timely patient records in electronic databases and medical record systems
- Complete required documentation, reporting, and billing activities
- Follow HIPAA regulations and all applicable laws, policies, and procedures
- Assist with referrals, diagnostic workups, and immunization services as needed
Community Engagement & Health Promotion
- Provide disease prevention and health promotion education in clinics, schools, homes, community events, and health fairs
- Conduct screenings and outreach activities as requested
- Participate in public health education and awareness initiatives
Professional Development & Quality Improvement
- Attend trainings, workshops, conferences, and CMH program updates
- Stay current on standards of care, best practices, and emerging treatments
- Participate in continuous quality improvement initiatives and customer satisfaction activities
- Assist with audits, chart reviews, and quality assurance processes
- Demonstrate cultural humility and professionalism in all interactions
Emergency Preparedness & Response
- Participate in emergency response activities, drills, and public health preparedness exercises
- Assist with mass dispensing, shelters, screenings, case investigations, and emergency response operations when needed
- Fulfill duties within the Incident Command System (ICS) structure
- Maintain flexibility for emergency response outside regular business hours
Additional Duties
- Provide seasonal assistance during periods of increased public health demand, including vaccination clinics and community outreach efforts
- Perform additional duties as assigned by the Nursing Supervisor or Director of Nursing
Education & Qualifications Required:
- Associate or Bachelor of Science in Nursing from an accredited nursing program
- Current Ohio nursing licensure
- Current CPR certification
- Valid Ohio Driver’s License with acceptable driving record and state minimum insurance
- Access to reliable transportation for community visits
Preferred:
- Public health nursing experience or related community health experience

Physical Demands & Work Environment
This position requires frequent sitting, standing, walking, speaking, listening, reading, and writing in English. Occasional lifting up to 25 pounds may be required. Duties include travel throughout the community for home visits, meetings, and outreach activities.
