Pennsylvania Association of Community Centers (PACHC) is seeking a driven, self-motivated Public Health Program Manager. This role will promote, support, and advance public health and population-focused initiatives across Pennsylvania’s community health centers. This role provides training and technical assistance, manages grant-funded programs, facilitates clinical work groups, and collaborates with state and national partners to advance data-informed, value-based, and prevention-oriented initiatives.

Key Responsibilities

Public Health and Population Health Initiatives

  • Collaborate with the Director of Clinical Operations and cross-functional team members to advance strategic public health and population-focused initiatives.
  • Lead or support the development and implementation of data-informed initiatives using performance metrics and health center data to identify needs and priorities.
  • Participate in and support the Value-Based Care Focus Group.
  • Translate data, regulatory requirements, and emerging trends into actionable strategies for health centers.

Oral Health Initiatives

  • Develop, implement, and coordinate training and technical assistance to support oral health integration within primary care settings.
  • Manage oral health-related grants and initiatives, including school sealant, workforce, and other partner-funded projects.
  • Facilitate and coordinate the Dental Work Group.
  • Build and maintain partnerships with key stakeholders, including state agencies, coalitions, and national oral health organizations.
  • Monitor trends, workforce issues, and policy developments affecting oral health programs.

Emergency Preparedness Initatives

  • Develop, implement, and coordinate emergency preparedness training and technical assistance for health centers.
  • Facilitate and coordinate the Emergency Preparedness Work Group.
  • Collect, analyze, and report emergency-related issues affecting health centers, including required reporting to HRSA and other entities.
  • Maintain partnerships with public health and emergency preparedness organizations.
  • Participate in relevant meetings and communicate important updates to internal and external stakeholders.

Maternal Health Initiatives

  • Serve as project manager for maternal health and family planning quality improvement initiatives, including collaboration with external partners.
  • Participate in meetings and work groups focused on maternal health.
  • Facilitate trainings and activities related to assigned maternal health initiatives.

Grant and Project Management

  • Manage multiple grant-funded programs simultaneously, ensuring compliance with funder requirements, deliverables, budgets, and reporting timelines.
  • Coordinate with leadership and finance staff to monitor budgets.
  • Track time and documentation across funding streams.
  • Prepare reports, summaries, and updates for leadership, funders, and partners.

Qualifications

  • Associates or Bachelor’s degree in Public Health, Health Administration, Nursing, or a related field; advanced degree such as MPH or MHA preferred.
  • Experience working with Federally Qualified Health Centers (FQHCs) or HRSA-funded organizations.
  • Experience supporting value-based care or population health initiatives.
  • Experience writing and managing grant applications and programs
  • Knowledge of FQHCs, health center clinical operations, and HRSA clinical-related regulations.
  • Understanding of Medicaid and Medicare programs.
  • Ability to manage multiple complex initiatives at the same time.
  • Advanced Excel skills, including data analysis and reporting.
  • Experience with performance dashboards and interpreting healthcare data.
  • Ability to translate data into actionable recommendations.
  • Excellent organization, prioritization, and problem-solving abilities.
  • Ability to collaborate effectively with clinicians, executives, and external partners.
  • Adaptability and sound professional judgment.
  • Ability to work successfully in a fast-paced, hybrid environment.

Pay: $65,000.00 - $72,000.00 per year
