Job Summary
Midwest Community Services is seeking a detail-oriented and driven Quality Assurance Coordinator to oversee quality assurance and compliance across both our disability services and foster care programs.
This role is responsible for creating and maintaining measurable weekly, monthly, and annual review systems, ensuring regulatory compliance, and driving continuous quality improvement across all service lines using systems such as CaseFlow, Therap, and Smartsheet.
What You’ll DoQuality Assurance & Compliance
- Monitor and ensure compliance with Nebraska DHHS (DD & CFS) regulations
- Conduct audits of client files, service documentation, and program operations within CaseFlow and Therap
- Review incident reports (GERs, critical incidents) in Therap and ensure proper documentation and follow-up
- Identify compliance risks and implement corrective action plans
Data Tracking & Reporting
- Build and maintain weekly, monthly, and annual QA tracking systems in Smartsheet
- Track key performance indicators (KPIs) such as:
- Documentation compliance
- Incident trends and response times
- Service delivery accuracy (authorized vs delivered)
- Staff training and credential compliance
- Maintain dashboards and reporting tools in Smartsheet for real-time visibility
- Extract and analyze data from CaseFlow and Therap to support reporting
- Prepare and present QA reports to leadership
Audits & Reviews
- Conduct weekly internal reviews for high-risk areas (documentation, incidents, safety concerns)
- Complete monthly audits across all programs using CaseFlow and Therap records
- Lead quarterly and annual quality reviews, including trend analysis
- Track and follow through on all audit findings using Smartsheet workflows
Process Improvement
- Develop and refine policies, procedures, and QA tools
- Standardize documentation practices across CaseFlow, Therap, and internal systems
- Partner with leadership to improve systems, workflows, and compliance outcomes
- Support implementation of new programs, services, and regulatory changes
Training & Support
- Train staff on documentation standards and proper use of:
- CaseFlow (CFS documentation and case tracking)
- Therap (GERs, T-Logs, service documentation)
- Smartsheet (QA tracking and workflows)
- Provide coaching and feedback based on audit results
- Support performance improvement efforts when needed
QualificationsRequired
- 2+ years of experience in:
- Quality assurance, compliance, or program oversight
- Human services, healthcare, disability services, or foster care
- Experience using or working within CaseFlow, Therap, or similar systems
- Strong attention to detail and organizational skills
- Ability to analyze data and identify trends
- Strong written and verbal communication skills
Preferred
- Direct experience with:
- CaseFlow (Nebraska CFS system)
- Therap documentation system
- Smartsheet for tracking and reporting
- Experience with Nebraska DHHS (DD Waiver and/or CFS)
- Bachelor’s degree in Social Work, Psychology, Healthcare Administration, or related field
Work Environment
- Fully onsite role based in Omaha, NE
- Regular interaction with staff, leadership, and program documentation systems
- Daily use of CaseFlow, Therap, and Smartsheet
- Occasional visits to program sites may be required
