The Quality Assurance Licensed Vocational Nurse (QA LVN) supports clinical excellence and quality assurance through detailed documentation review and quality oversight. This role is key to maintaining our 5-Star Medicare rating and ensuring regulatory compliance.
This is a mostly remote position — with occasional on-site training and meetings in Tyler, Texas. Must reside within one hour of the Tyler, TX office.
Qualifications

Active Texas LVN license

Minimum 3 years of healthcare experience, including 1 year in Home Health

Strong attention to detail and clinical accuracy

Skilled in organization, problemsolving, and communication

Must reside within 75 miles of the Tyler, TX office
Key Responsibilities

Review clinical documentation for accuracy, completeness, medical necessity, and compliance

Validate OASIS data behind an external OASIS review company and resolve alerts or edits

Review diagnosis coding for alignment with documentation

Collaborate with Clinical Teams to improve care quality and consistency

Educate clinicians on documentation accuracy and regulatory requirements

Provide ongoing training and coaching to support quality performance

Participate in QAPI activities and agency performance improvement initiatives

Partner with the QA Manager, Vice President of Quality and Compliance, and Executive Director on continuous quality improvement
