Position Summary

Romeo Staffing Agency is seeking a highly motivated and detail-oriented Quality Assurance Registered Nurse (QA RN) to join our Clinical Quality Department.

The QA RN will be responsible for reviewing and auditing clinical documentation submitted by U.S.-based clinicians to ensure accuracy, completeness, medical necessity, and compliance with Medicare, CMS, state regulations, and agency policies.

This position plays a critical role in improving documentation quality, reducing compliance risks, and supporting excellent patient care.

Key Responsibilities

  • Review and audit clinical documentation for accuracy, completeness, and regulatory compliance.
  • Perform quality reviews of:
  • Skilled Nursing Visit Notes
  • Start of Care (SOC)
  • Resumption of Care (ROC)
  • Recertification (RECERT)
  • Discharge Assessments
  • Evaluation Notes
  • Routine Visit Notes
  • OASIS Documentation (preferred)
  • Verify documentation supports medical necessity and physician orders.
  • Identify missing information, inconsistencies, documentation errors, and compliance concerns.
  • Communicate documentation deficiencies professionally with clinicians and internal departments.
  • Ensure documentation complies with:
  • CMS Guidelines
  • Medicare Conditions of Participation (Cop's)
  • Home Health regulations
  • Agency policies and procedures
  • Review physician orders for accuracy and completeness.
  • Monitor documentation turnaround times and follow agency timelines.
  • Maintain accurate QA tracking logs and audit reports.
  • Participate in Quality Improvement (QI) initiatives.
  • Assist with clinical process improvements and documentation education.
  • Support internal audits and compliance reviews.
  • Collaborate with Intake, Patient Navigation, Scheduling, Medical Records, Billing, and Clinical Management teams.
  • Escalate significant compliance concerns to the Clinical QA Manager when appropriate.
  • Maintain strict HIPAA compliance and patient confidentiality.

Required Qualifications

  • Bachelor of Science in Nursing (BSN/BScN) from a recognized institution.
  • Valid Pakistan Nursing Council (PNC) license.
  • Minimum 1–2 years of clinical nursing experience.
  • Experience in Quality Assurance, Medical Documentation Review, Clinical Auditing, or Home Healthcare is preferred.
  • Excellent understanding of nursing documentation standards.
  • Strong analytical and critical-thinking skills.
  • Exceptional attention to detail.
  • Strong written and verbal English communication skills.
  • Ability to interpret clinical documentation accurately.
  • Strong organizational and time-management skills.
  • Ability to work independently while collaborating within a multidisciplinary team.
  • Willingness to work permanent night shifts aligned with U.S. business hours.
