Rapid Rehousing Lead Case Manager

We are seeking a dedicated and experienced Rapid Rehousing Manager Lead to support individuals and families transitioning from crisis housing into permanent, stable housing. This role combines direct service delivery with team leadership, ensuring high-quality, client-centered case management services.

ESSENTIAL FUNCTIONS

Client Support & Case Management

  • Conducts intakes, assessments, and develop individualized service and housing stability plans
  • Provides ongoing case management, crisis intervention, and goal tracking
  • Connects participants to community resources including employment, mental health, and education services
  • Delivering life skills training such as budgeting, tenant responsibilities, and independent living

Housing Navigation

  • Assists clients in locating and securing safe, affordable housing
  • Supports lease negotiations and coordinate move-in processes
  • Acts as a liaison between tenants and landlords to ensure housing stability
  • Builds partnerships with landlords and housing providers

Leadership & Program Support

  • Supervises, coaches, and supports Rapid Rehousing Case Managers
  • Monitors case files and data entry for compliance and quality
  • Collaborate with program leadership and cross-functional teams
  • Contributes to program development, reporting, and team initiatives

REQUIRED EDUCATION AND EXPERIENCE

Required

  • Bachelor's degree in social services, Human Services, Public Administration, or related field (or equivalent experience)
  • Minimum 1 year of experience in social services, preferably in housing or homelessness programs
  • Strong communication, organizational, and problem-solving skills
  • Experience working with diverse and underserved populations

Preferred

  • Experience with housing navigation or rapid rehousing programs
  • Knowledge of housing systems, employment resources, and community services

Additional Requirements

  • Valid driver’s license and ability to travel locally
  • Ability to pass a background check
  • Completion of required trainings (e.g., CPR/First Aid, Trauma-Informed Care, HIPAA) within 90 days
