At Yorhealth we provide care for children with complex clinical and behavioural needs. In partnership with members of the multidisciplinary team the post holder will provide a seamless complex care service, ensuring children receive the highest standard of clinical support.
We are looking for an experienced Paediatric Nurse to join our growing team in the Nottingham area on a part time basis 3 days per week, and can offer a comprehensive range of benefits including ongoing professional development and supportive working environment making this an attractive opportunity for an experienced children’s nurse.
The role will be home-based, where you will manage your own diary to ensure the needs of the client and business are met (Monday-Friday). The area you will cover will be within the Nottingham and Doncaster areas .
Job Overview
This is an exciting opportunity for a qualified Children’s Nurse with excellent clinical skills to lead a core team of carers and provide face-to-face support to children and their families. The successful candidate will create personalised care plans for each child and work closely with the team to ensure seamless delivery of care.
Your role will include:
- Completing clinical assessments, care planning, Risk Assessments, monthly audits and care package reviews
- Completing and investigating incident reports where appropriate, with the support of the Senior Nurse Manager and the Head of Services/Registered Manager
- Work effectively with the wider nursing and administrative team to source, recruit and develop a wide ranging and varied complex care team to meet the needs of each individual client. Ensuring the team members are skilled, confident and well supported to deliver excellent care.
- Provide on call cover for a week on a rotational basis for emergency call handling for out of hours clinical advice and support. Additional payment is given for this.
- Participate in the planning and delivery of formal and informal education programmes for clients, carers and all appropriate staff
- Ensure that professional knowledge and practice is constantly updated
- With support, manage a caseload based on flexible principles and prioritise the care needs of clients
- Provide nursing advice and support to the team and their families throughout the care pathway, utilising their skills of clinical expertise, education, and management to ensure a seamless service
- Work closely with the multidisciplinary team in hospital and the community to ensure a co-ordinated quality service
- Engage and lead discussions in regularly scheduled commissioner and multidisciplinary team meetings
- Communicate and liaise with clinical managers to achieve effective knowledge sharing and appreciation of all complex care activity.
- Attend weekly case management meetings to keep the team updated on their care package and provide support to other members of the team when needed.
