Freedom Senior Services is seeking a skilled and compassionate Remote Registered Nurse (RN) to join our growing telehealth team. In this role, you will provide virtual clinical assessments, triage, patient education, and ongoing care coordination using secure digital platforms. You will support seniors and their families by delivering professional, empathetic, and accurate guidance that aligns with our mission of exceptional senior care.
Conduct remote patient triage via phone or video, assessing symptoms, risk factors, and care needs.
Perform virtual nursing assessments using guided questioning, visual observation, and patient‑reported health data.
Provide clear medical guidance, health education, and instructions for treatment plans and follow‑up care.
Support chronic disease management through scheduled telehealth visits, reviewing labs, medications, and symptom trends.
Document all patient interactions, assessments, and care plans accurately within electronic health record systems.
Collaborate with physicians, care coordinators, and interdisciplinary teams to ensure comprehensive patient care.
Monitor patient progress and respond to alerts from remote patient monitoring tools.
Offer emotional support and counseling to patients and families in a virtual environment.
Maintain strict adherence to HIPAA, privacy standards, and Freedom Senior Services’ clinical protocols.
Active RN license (state‑specific; multi‑state NLC license preferred).
ADN or BSN degree; BSN strongly preferred.
Minimum 1–3+ years of clinical nursing experience (acute care, primary care, geriatrics, or telehealth).
Strong assessment, triage, and patient‑education skills suitable for virtual care.
Proficiency with telehealth platforms, EHR systems, and remote monitoring devices.
Excellent communication skills, with the ability to build rapport and trust through virtual interactions.
Reliable high‑speed internet and a secure, private workspace.
Experience in senior care, chronic disease management, or telephone triage.
Familiarity with digital health workflows and remote patient monitoring.
Ability to interpret patient‑generated health data and identify concerning trends.
Strong critical‑thinking and problem‑solving abilities.
Fully remote position with flexible scheduling options.
Requires consistent availability for virtual patient visits, documentation, and team collaboration.
Must maintain a professional, quiet, and secure home office environment.
