,Job Title: Healthcare Coordinator – Registered Nurse (RN)

Alternative Supports, Inc is a non-profit human services agency that is committed to improve the quality of life and to support and develop the functioning of individuals intellectual disabilities through meaningful social opportunities, positive therapeutic experiences, learning environments, natural supports, and enriched living arrangements with a commitment to on-going monitoring and evolutionary changes.

Alternative Supports, Inc (ASI) is seeking a highly organized and proactive individual to join our team as a Healthcare Coordinator. As a Healthcare Coordinator, you will play a crucial role in supervising, training, and evaluating staff members to ensure the medical needs of individuals served are in place.

Position Location: This position will be traveling between our residential programs in Billerica, Dracut, Chelmsford, Tyngsborough, Lowell, and Tewksbury.

Schedule: Monday to Friday, 9:00 AM to 5:00 PM, will be required to flex hours at times to accommodate the needs of the agency.

Duties and Responsibilities (These duties are a general summary and not all-inclusive)

  • Works with the Director of Health Services and all other ASI Team members to ensure continuity of care.
  • Participate in and attend Human Rights Committee Meetings Quarterly as needed.
  • Participate in medical aspects of the admissions, discharge, or program transfer process as needed including but not limited to health care reviews.
  • Assist the Director of Health Services with the development, implementation, and evaluation of health/medical care department goals and the tracking of documents.
  • Provide hands-on training, guidance, and support to Support Coordinators, and Direct Care Staff in the areas of medical knowledge.
  • Obtain and Maintain Medication Administration Program (MAP) trainer certification.
  • Responsible for monitoring medication administration, and on-going health status of individuals served.
  • Ensure that program sites are compliant with State and Federal regulations regarding health and safety.
  • Communicate health care information to all key team members.
  • Under the supervision of the Director of Health Services, assess, plan, implement, and evaluate health care services related to individual needs.
  • Attend medical appointments and communicate with providers as needed.
  • Adhere to the agency’s mission, policies, and procedures.
  • Carry out all activities in accordance with the agency’s policies, procedures, and objectives and supervisory directions.

Qualifications:

  • Must be currently licensed in Massachusetts in accordance with applicable law as a Registered Nurse (RN).
  • Has at least two years of training experience.
  • Three years of nursing experience, working with individuals with developmental disabilities is strongly preferred.
  • Knowledge of Department of Developmental Services (DDS) and Department of Public Health (DPH) regulations is preferred.
  • Excellent interpersonal, critical thinking, problem-solving, and communication skills.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.
  • Possess a valid driver’s license.
  • Proficiency in Microsoft Office Suite.
  • Must be able to communicate effectively with others and individuals served.
  • Clearance through state mandated Background/Fingerprint Check(s).

Job Type: Full-time

Pay: From $89,440.00 per year
