The Registered Nurse (RN) provides skilled nursing care and compassionate support to clients in their homes or hospice settings. The RN is responsible for assessing, planning, implementing, and evaluating individualized care plans to ensure each client receives the highest standard of comfort, dignity, and medical care.
Clinical Care
Perform comprehensive nursing assessments and develop individualized care plans.
Administer medications, treatments, and procedures as prescribed by the physician.
Monitor and document vital signs, symptoms, and client progress.
Provide wound care, IV therapy, catheter care, and other skilled nursing services.
Coordinate with physicians, caregivers, and other healthcare professionals to ensure continuity of care.
Respond promptly to changes in a client’s condition and update care plans accordingly.
Hospice & Palliative Care
Provide end-of-life support focusing on comfort, pain management, and emotional well-being.
Educate family members on the hospice process, comfort measures, and medication administration.
Offer emotional support and compassionate communication to families during the hospice journey.
Client & Family Education
Teach clients and families about disease management, medication schedules, and ongoing care needs.
Promote independence and quality of life through education and advocacy.
Documentation & Compliance
Accurately document assessments, interventions, and outcomes in the electronic medical record (EMR).
Ensure compliance with state, federal, and agency regulations.
Maintain confidentiality and uphold all HIPAA standards.
Current and valid Registered Nurse (RN) license in the state of practice.
Minimum of 1–2 years of nursing experience (home health or hospice preferred).
Current CPR certification.
Strong clinical assessment and critical thinking skills.
Compassionate, dependable, and able to work independently in clients’ homes.
Valid driver’s license, reliable transportation, and proof of insurance.
Flexible schedule: full-time, part-time, or PRN options available.
Care is provided in private homes, assisted living, or hospice settings.
Requires local travel between client locations.
