Overview
Join our dynamic healthcare team as a Registered Nurse Case Manager, where you will play a vital role in coordinating comprehensive patient care. This energetic position offers the opportunity to utilize your critical care expertise. Ideal candidate will need to demonstrate good leadership, communication, and writing skills. Demonstrate proficiency in the use of Oasis to develop optimal level of patient care under physician orders. Enforce the policies and procedures of the agency. Follows Medicare and Medicaid CoPs guidelines.
Duties
- Perform admission, transfer, re-certification, resumption of care, and discharge Oasis for the home care patient.
- Initiate the plan of care under doctor’s orders.
- Educate patients and caregivers on disease process, medications, plan of care, and individualized treatment plans.
- Engage in case management activities.
- Monitor patient progress through ongoing assessment of vital signs and clinical indicators and adjust plan of care proactively.
- Collaborate with other member of the healthcare team within agency (PT/OT/SP/SW).
- Maintain detailed medical records and documentation of all interactions while adhering strictly to HIPAA privacy standards.
- Serve as a liaison between patients, families, healthcare teams, and community resources to ensure comprehensive support throughout the continuum of care.
- Participate in ongoing continue educations in accordance with the condition of Participations (CoPs).
Qualification:
- Ability to pass fingerprint background check and drug screen to be considered for this position.
- One years of home healthcare and long-term-care experience required for this position.
- Be a registered nurse (R.N.) with unrestricted license in the state of Ohio.
- Work positively and favorably with patients, families, and staff.
- Good communication and time management skills.
- Good attitude and excellent work ethics.
- Reliable transportation needed for this position.
