Overview

Duties

  • Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using EMR (Electronic Medical Record) systems such as Therap.
  • Administer medications and medication management accurately following HIPAA guidelines and aseptic techniques.
  • Provide specialized care such as wound care, tube feeding, and catheterization procedures.
  • Assist in diagnostic evaluations and ensuring proper documentation.
  • Manage emergency situations with skills in decision making for medical processes and procedures
  • Educate staff, patients, and team members about healthcare coordination and plans .
  • Coordinate internal case management activities to ensure seamless transitions across inpatient, outpatient, home care, or hospice settings.

Requirements

  • Valid RN license with current registration in the applicable state or jurisdiction.
  • Proven experience working in diverse clinical environments such as hospital medicine, ICU (Intensive Care Unit), emergency room (ER), pediatrics, geriatrics or behavioral health settings.
  • Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/ICD-10), CPT coding for procedures, and medical documentation standards.
  • Proficiency with EMR/EHR systems like Therap; or willing to learn
  • Excellent patient assessment skills with a focus on infection control practices and aseptic techniques; experience with infection control protocols is desirable.
  • Strong communication skills for triage assessments and working effectively with individuals with disabilities or developmental challenges.
  • Knowledge of HIPAA regulations ensuring confidentiality of patient information at all times.
