About Us

Trinity’s Care LLC is a Michigan-based provider of Adult Foster Care (AFC) and Home Help services dedicated to delivering quality care, dignity, and independence to individuals in community-based settings. We are seeking a compassionate and experienced Registered Nurse (RN) to provide clinical oversight, resident assessments, staff training, and compliance support in accordance with Michigan Adult Foster Care regulations and Area Agency on Aging requirements.

Position Summary

The Contract RN will provide professional nursing oversight for AFC residents and Home Help clients twice a year. The RN will complete comprehensive resident assessments, develop and review care recommendations, conduct required staff training, and ensure compliance with state regulations and funding source requirements.

Essential Duties and Responsibilities

Resident Assessments

• Conduct initial nursing assessments for AFC residents.
• Complete semi-annual (twice yearly) resident health and functional assessments.
• Review medical diagnoses, medications, and healthcare needs.
• Identify changes in resident conditions and recommend interventions.
• Document findings and maintain compliance records.

Staff Training and Supervision

• Conduct mandatory caregiver training at least twice annually.
• Maintain training records and attendance logs.

Clinical Oversight

• Review resident care plans and service plans.
• Provide recommendations regarding resident care needs.
• Consult with physicians, case managers, guardians, and other healthcare professionals as needed.
• Participate in quality assurance activities.
• Assist with licensing compliance and surveys.

Documentation

• Complete assessment reports in a timely manner.
• Maintain accurate nursing records.
• Submit required reports to Trinity’s Care LLC management.

Qualifications

• Current unrestricted Michigan Registered Nurse (RN) license.
• Minimum two years of nursing experience preferred.
• Experience in AFC, long-term care, home care, behavioral health, geriatrics, or developmental disabilities preferred.
• Knowledge of Michigan AFC licensing requirements.
• Strong assessment, communication, and documentation skills.
• Current CPR certification preferred.
• Professional liability insurance preferred.

Schedule

• Flexible schedule.
• Approximately 2–8 hours monthly depending on resident census.
• Semi-annual resident assessments required.
• Semi-annual staff training sessions required.

Contractor Requirements

• Maintain active Michigan RN license.
• Pass background screening if required.
• Reliable transportation.
• Ability to travel to AFC locations within Berrien County and surrounding areas.
