Registered Nurse Case Manager

The Hospice Registered Nurse (RN) provides compassionate, skilled nursing care to hospice patients and their families, focusing on comfort, symptom management, and quality of life in accordance with the interdisciplinary plan of care.

Responsibilities:

• Perform initial and ongoing patient assessments
• Develop and update individualized plans of care with the IDT
• Provide skilled nursing care, pain and symptom management
• Administer medications and treatments per physician orders
• Educate patients and caregivers on care, medications, and disease progression
• Coordinate care with physicians and interdisciplinary team members
• Complete accurate and timely clinical documentation
• Participate in on-call rotation as assigned

Qualifications:

• Current, unrestricted RN license
• Minimum 1 year nursing experience (hospice/home health preferred)
• CPR certification
• Valid driver’s license and reliable transportation
• Ability to meet health, background, and compliance requirements

Pay: $70.00 - $90.00 per hour
