Registered Nurse Case Manager

Job Overview
We are seeking a dedicated and compassionate Registered Nurse to join our healthcare team. The ideal candidate will possess strong clinical skills and a commitment to providing high-quality patient care.

This job will mainly involve assisting providers with refill requests, triaging patients and providing medical advice, helping coordinate care with specialists and after ED or hospital discharge. This is mainly an administrative position, with 1-2 days in clinic per week and 2-3 days work from home.

Duties

• Work with providers regarding patient care, refill requests, phone triage.
• Administer medications and treatments as prescribed while monitoring patient responses.
• Collaborate with interdisciplinary teams to ensure coordinated care and optimal patient outcomes.
• Provide health coaching and education to patients and their families regarding health management.
• Maintain accurate medical documentation in compliance with ICD-10 coding standards.
• Participate in case management activities to facilitate effective discharge planning.
• Engage in continuous professional development through training and education opportunities.

Experience
Candidates should have a valid nursing license and one year relevant experience in a clinical setting. Familiarity with laboratory procedures, ambulatory surgery centers, health coaching techniques, medical documentation practices, case management systems is highly desirable. Strong communication skills, attention to detail, and the ability to work effectively in fast-paced environments are essential for success in this role.
