Key Responsibilities

• Perform medical necessity screenings and submit for Physician Advisor review as needed.
• Coordinate patient care and transition planning assessments and reassessments.
• Lead and facilitate multidisciplinary patient care conferences.
• Manage concurrent disputes and make referrals to appropriate departments or social services.
• Collaborate with physicians, office staff, and ancillary departments to ensure continuity of care.
• Educate patients and families regarding the plan of care and post-discharge needs.
• Maintain accurate and timely documentation in the Case Management system.
• Ensure compliance with regulatory standards, hospital policies, and accreditation requirements.
• Participate in utilization management and transition management to promote efficiency and patient satisfaction.

Requirements

Shift: Day Shift, 5x8s (0800–1630), rotating days

Education & Experience:

• Graduate of an accredited school of nursing — Required
• Two (2) years of acute care RN experience — Required
• One (1) year of hospital experience — Preferred
• Experience in utilization review, discharge planning, and/or home health — Preferred
• Recent acute care hospital case management experience — Preferred

Licenses & Certifications:

• Current Registered Nurse (RN) license in the state of practice — Required
• Accredited Case Manager (ACM) certification — Preferred

Pay: $75.00 - $80.00 per hour
