Coverage Areas: Newton Needham Waltham Watertown (Clinicians not expected to cover all listed areas. Accepted areas are broken into territories)

What is the role of Case Manager?

We are searching for a Skilled Nurse to support and manage our patients in the Greater Boston area. You must be known to be warm, extremely focused, professional, compassionate, communicative, talented, experienced, and reliable.

Your day will include:

• Providing skilled nursing care to adult patients in a home setting
• Implement, develop, and document the plan of care to ensure quality and continuity of care for each patient with admissions, recerts and resumptions of care and supervisory visits.
• Provide effective communication to patient/family, team members, and other caregivers involved
• Implement plan of care and treatment using sound judgment and critical thinking
• Provide accurate, detailed, and timely documentation consistent with the plan of care and the nursing process
• Administer prescribed medications and treatments, per MD order and Participate in interdisciplinary staff meetings to exchange ideas and information as it relates to the care and quality initiatives.

Qualifications:

• Current Registered Nurse in the state of MA
• Requires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency.
• Knowledge of HIPAA regulations
• Current CPR accredited by American Heart Association and/or American Red Cross (certificate must be healthcare provider or healthcare professional).
• Valid MA Driver’s license

Pay: $85,000.00 - $100,000.00 per year
