Registered nurse home health

Overview
We are seeking a dedicated and compassionate Registered Nurse for our Home Health department. This role involves providing high-quality nursing care to patients in their homes, ensuring their comfort and well-being while promoting independence. The ideal candidate will possess a diverse skill set, with experience in various nursing specialties, and a commitment to delivering exceptional patient care.

Duties

• Conduct comprehensive patient assessments and develop individualized care plans.
• Monitor patient vital signs and health status, documenting changes accurately in electronic medical records (EMR).
• Provide wound care, catheterization, and ostomy care as needed.
• Collaborate with interdisciplinary teams to coordinate patient care and discharge planning.
• Educate patients and families on health management, medication administration, and rehabilitation techniques.
• perform, admission assessments,recertification, resumption of care assessment

Qualifications

• Current Registered Nurse (RN) license in the state of practice.
• Experience in home health care or similar settings preferred.
• Strong clinical skills including but not limited to IV insertion, phlebotomy, triage, and medication administration.
• Familiarity in completion of oasis assessments
• Experience in critical care, emergency medicine, geriatrics, or hospice & palliative medicine is a plus.
• Ability to work independently while managing multiple priorities effectively.
• Excellent communication skills for interacting with patients, families, and healthcare teams. Candidates with experience in specialized areas such as pediatrics, behavioral health, pain management, or trauma medicine will be considered favorably. Job Type: Part-time

Pay: $80.00 - $250.00 per hour

Expected hours: 8 – 24 per week
