Registered Nurse Home Health Experience

Job Summary
We are seeking a dedicated and compassionate Registered Nurse with home health experience to join our dynamic healthcare team. In this vital role, you will provide comprehensive nursing care directly in patients’ homes, ensuring personalized treatment plans and promoting optimal health outcomes. Duties

  • Conduct thorough patient assessments, including vital signs, physical examinations, and detailed health histories to develop individualized care plans.
  • Administer medications via IV infusion, injections, and other routes, ensuring proper documentation and adherence to safety protocols.
  • Provide specialized care for patients with complex needs such as wound management, ostomy care, catheterization, and pain management.
  • Manage acute conditions including respiratory distress, infectious disease care, and trauma-related injuries within the home setting.
  • Educate patients and their families on disease management, medication administration, nutrition, and safety measures tailored to their specific health conditions.
  • Coordinate with interdisciplinary teams for discharge planning, case management, and referrals to community resources or specialty services like ophthalmology or geriatrics.
  • Maintain accurate medical records using EMR (Electronic Medical Records) systems such as Wellsky, Epic, or Cerner while complying with HIPAA regulations.

Qualifications

  • Valid RN license with current registration; prior home health experience strongly preferred.
  • Demonstrated proficiency in medical documentation and medical terminology; familiarity with ICD-10 coding is a plus.
  • Experience working with diverse patient populations including pediatric (Toddler care), geriatric (Geriatrics), and individuals with disabilities or developmental delays.
  • Skilled in IV insertion, phlebotomy, aseptic technique, wound suturing, and sterile processing procedures.
  • Knowledge of critical care practices such as ICU experience or Level I/II trauma center background is advantageous.
  • Ability to perform comprehensive patient assessments including vital signs, physiology knowledge, and diagnostic evaluation.
  • Strong communication skills for triage situations and patient education; familiarity with EMR/EHR systems like Athenahealth or eClinicalWorks preferred.
