Full-Time Registered Nurse - Home Health (Psych Focus) – Downtown Boston

Salary: $70K–$90K base, with potential to earn $100K+

We are looking for a full-time Registered Nurse for one of our partner organizations. This is a salaried, direct-hire role, and the selected nurse will become a full-time employee eligible for the organization’s full benefits package. About the Role

• Home health, psych focus
• Monday - Friday
• Evening shift
• Majority of patients are in a downtown Boston shelter and must be seen between 7pm and 9pm
• A small number of additional patients are located nearby
• Most visits are medication management and take about 10–15 minutes
• Weekly productivity expectations
• Uncapped incentive pay for visits above productivity, with the opportunity to significantly increase earnings

Key Responsibilities

• Provide skilled nursing care and medication management
• Perform assessments and monitor changes in condition
• Communicate with providers and adjust care plans as needed
• Educate patients on medications, chronic conditions, and self-care
• Provide health coaching and preventative guidance
• Maintain long-term patient relationships
• Complete documentation efficiently with remote charting

Requirements

• Active Massachusetts RN license
• Minimum one year of nursing experience (home care preferred)
• Strong clinical judgment and communication skills
• Reliable transportation for in-home visits
