Home Health RN – Orange County
Assignment Type: 13-week contract with potential to extend
Schedule: Monday–Friday, 8:30 AM–5:00 PM + 1 weekend per month

Job Overview

Clover Health Services is seeking an experienced Home Health Registered Nurse to support patient care throughout Orange County. This role involves completing home visits, managing a caseload using the point system, and providing high-quality skilled nursing services.

Responsibilities

• Conduct patient home visits (goal: 25 points per week)
• Routine visits = 0.75 points
• Admissions = 2.5 points
• Provide skilled nursing care including wound care, IV therapy, and case management
• Complete accurate and timely OASIS documentation
• Participate in 1–2 on-call shifts as needed
• Collaborate with the interdisciplinary team to support patient outcomes

Requirements

• Active RN license (New York)
• Certified Home Health Agency (CHHA) experience required
• OASIS proficiency required
• Strong background in case management, wound care, and IV procedures
• Knowledge of Homecare Home Base (HCHB) strongly preferred
• BLS certification required

Compensation

• Travel Weekly Pay-$3631, Blended Hrly Pay-$90.78
• Local Weekly Pay-$3409, Hrly Pay-$85.23
• Facility-paid mileage: $0.50/mile (between patient visits only)

Orientation

• 2–7 days based on experience
• Training provided for triaging and workflow systems
